This website, utahstemcelltherapy.com (the “Site”), is operated by Integrated Pain Specialists (“we,” “us,” or “our”), a pain management practice with clinics in Bountiful and South Jordan, Utah, and a surgical center in West Jordan, Utah. This Privacy Policy explains what information we collect through the Site, how we use it, who we share it with, and the choices you have. By using the Site, you agree to the practices described here.
This Policy covers the Site only. Once you become a patient, the medical information we hold about you is protected under the Health Insurance Portability and Accountability Act (HIPAA) and is governed by our Notice of Privacy Practices, which is available at any of our clinics.
Information you give us. When you submit the consultation request form, we collect the information you enter: your first and last name, email address, phone number, the location you prefer to visit, the general area of your body that is bothering you, and anything you choose to write in the message field. Submitting the form is voluntary, and you can contact us by phone instead.
Information collected automatically. Like most websites, our hosting provider records standard technical information when you visit, including your IP address, browser type, device type, the pages you view, the time of your visit, and the site that referred you. If we use analytics tools (see “Cookies and analytics” below), those tools collect similar information to help us understand how the Site is used.
Information we do not collect. The Site does not process payments, does not ask for insurance numbers, Social Security numbers, or dates of birth, and does not host patient records. Please do not use the form to send detailed medical history, imaging, or urgent medical concerns. If you have an urgent medical need, call 911 or go to the nearest emergency room.
We use the information you provide to respond to your consultation request, schedule and confirm your visit, answer your questions, and communicate with you about your request. By giving us your phone number, you agree that we may call or text you about your request; you can ask us to stop at any time. We use technical and analytics information to keep the Site secure, fix problems, and improve the content and performance of the Site.
We do not sell your personal information, and we do not use the information you submit through the Site for advertising to third parties.
We share information only as needed to run the Site and respond to you. That includes service providers who host the Site, deliver email, and help us manage appointment requests and communications. These providers may use your information only to perform services for us. We may also share information when required by law, to respond to a lawful request from a government authority, to protect the rights, safety, or property of our patients, staff, or the public, or in connection with a merger, sale, or reorganization of our practice, in which case we will require the new owner to honor this Policy.
The Site may use cookies and similar technologies, including Google Analytics, to measure traffic and understand how visitors use the Site. These tools set cookies in your browser and collect information such as the pages you visit and the time you spend on them. You can control cookies through your browser settings, and you can opt out of Google Analytics using the browser add-on Google provides. Turning off cookies will not affect your ability to submit the consultation form.
Information you submit through the Site before you become a patient is not part of a medical record and is not covered by HIPAA. We treat it with care regardless: it is delivered to our staff over an encrypted connection, is used only to respond to your request, and is not shared for marketing. If you would rather not share the area of your body that is bothering you through the form, choose the closest general option or call the office nearest you.
We keep consultation requests for as long as needed to respond to you and to maintain a record of our communication, and then in accordance with our record-keeping practices and applicable law. We use reasonable administrative, technical, and physical safeguards to protect the information we collect, including encrypted connections (HTTPS) and access limited to staff who need it. No method of transmission over the internet is completely secure, so we cannot guarantee absolute security.
You may ask us to access, correct, or delete the personal information you submitted through the Site, and you may ask us to stop contacting you about your request, by calling any of our offices listed below. We will respond within a reasonable time and may need to verify your identity before acting on a request. Requests about your medical record as a patient are handled under our Notice of Privacy Practices rather than this Policy.
The Site is intended for adults. We do not knowingly collect personal information from anyone under 18 through the Site. If you believe a minor has submitted information to us, please contact us and we will delete it.
The Site links to our main practice website and may link to other sites, including government and public health resources. Those sites have their own privacy policies, and we are not responsible for their practices.
We may update this Policy from time to time. When we do, we will change the effective date at the top of this page. Continued use of the Site after a change means you accept the updated Policy.
